There are 6 (six) different forms in the kit namely:

  1. The New Account Opening form
  2. Indemnity Form
  3. Certificate Verification Form
  4. Share Allotment Form
  5. Sale Order Form
  6. Purchase Order Form.

 

New Account Opening Form

When you have downloaded this form, please complete the Individual Section for an Indivudual. If the Account is meant for your Company or Organisation, please complete the Corporate Section on page one. Please also select the account type you want (for details of account types, click >here. On page two of the form, please append your signature once for indivudual account holders and two different signatories for the Corporate account. The Documentation check list is also important. Please make sure all the items requested are included when mailing the documents too us.


 

The Indemnity Form

Please read the document properly and make sure you understand and agree with the content therein. Append your signature at the end in the space provided. This form when completed gives us the power to act on your instructions you have provided verbally, e-mail, Telephone and/or Facsimile.

Certificate Verification Form

This form is for the purpose of verifying the authencity of the certificates at the Registrars. We only require you to sign the form as the TRANSFEROR. Please note that it is one form per certificate so if you have 10 certificates you need to verify, you wil have to sign on 10 different forms. For Corporate Organisations, Please sign beside each other making sure that the signatures do not overlap.

Share Allotment Form

This Form is used for alloting to you the number of stocks you have purchased on the floor of the Nigerian Stock Exchange by the CSCS (Central Securities and Clearing Systems). Without this form, the CSCS would not transfer the purchased stock to your acoount with them. You are therefore required to just append your signature onto the signature space (the grey area), Individual account holders will sign once and corporate account holders will provide the combination signature on the two spaces provided in the grey area. You need one allotment form for each type of stock purchased (if you bought shares in 3 different companies at once, you will need to provide 3 allotment forms).

Sale Order Form

This form when you complete it and mail to us gives us your direction to sell the stock you have listed. Your order will subsequently be executed according to the instructions. Your order will not be executed if you do not append your signature to the form or if your signature is not regular or identical to the one on your account opening form.

Purchase Order Form

This form when you complete it and mail to us gives us your direction to buy the stock you have listed. Your order will subsequently be executed according to the instructions.
